Can I appeal an IRMAA decision?

Accepted Answer

Yes. CMS recognizes eight life-changing events that qualify you to use a more recent year's income instead of the 2-year-old number: marriage, divorce, death of a spouse, work stoppage or reduction, loss of pension income, employer settlement payment, or loss of income-producing property. File Form SSA-44 with documentation. A one-time Roth conversion or capital gain is not a qualifying event — you can't appeal those.

Does IRMAA apply if I have Medicare Advantage?

Yes. IRMAA is a surcharge on the underlying Part B (and Part D) premium, and Medicare Advantage members still pay Part B. The Part D piece applies if your Medicare Advantage plan includes drug coverage (MA-PD), or if you have a separate Part D plan.

Does a Roth conversion increase MAGI for IRMAA?

Yes — every dollar converted is taxable income and counts toward MAGI in the year of the conversion. That MAGI sets your IRMAA two years later. Many people stack conversions in their early 60s precisely to finish before the IRMAA window starts, or split conversions across calendar years to stay under a bracket cliff.

When does IRMAA actually start?

The month you (or your spouse) become enrolled in Medicare Part B and/or Part D. Most people are first hit at age 65. SSA notifies you each fall by letter showing the next year's premium.

What happens to IRMAA when a spouse dies?

Accepted Answer

The surviving spouse moves from joint thresholds to single thresholds — which are roughly half as wide. The same MAGI that was comfortably in tier 1 jointly may sit two tiers higher single. Death of a spouse is an appealable life-changing event, so file SSA-44 promptly if income hasn't changed.

Do HSA contributions or QCDs reduce my MAGI?

Accepted Answer

Both reduce AGI, and since MAGI for IRMAA = AGI + tax-exempt interest, they reduce your IRMAA MAGI too. Qualified Charitable Distributions (QCDs) from an IRA after age 70½ are especially powerful — they satisfy RMDs without adding to AGI.

I heard about a "hold harmless" rule. Does it help me?

The hold-harmless provision prevents Part B premium increases from reducing your Social Security check year-over-year — it does not exempt you from IRMAA. Higher-income beneficiaries (anyone subject to IRMAA) are explicitly excluded from hold harmless.
